In-Home Primary Care & Transitional Care Visits service provides a less complicated and more attainable route for chronic care management. We are now offering in-home visits for patients who may find it increasingly difficult to leave their home due to health conditions after hospitalizations. The nurse practitioner will fulfil the PCP role until the patient is able to transition back to seeing his/her healthcare provider in the office setting.
