General Healthcare-in-home Primary Care Service

The General Healthcare- In-Home Primary Care service provides a less complicated and more attainable route for chronic care management. We are now offering in-home visits for patients who may find it increasingly difficult to leave their home due to health conditions. The nurse practitioner will become the patient's primary care provider (PCP) or fulfil that role until the patient is able to transition back to seeing his/her healthcare provider in the office setting.
