SCHEDULING SELF-PAY APPOINTMENTS
How to Schedule Appointment
1)Specify whether you are a New or Established patient.
2)Select the appropriate appointment type from the drop down list.
If you are unsure of the appointment type, please read the appointment type description prior to scheduling.
- In the notes section, please state any additional information you want us to know for your appointment.
How to Make Appointment Payment
NEW PATIENTS:
1) After scheduling your appointment, return to the website and select the corresponding payment link for your visit type on the service type page.
2) Complete payment within 2 hours of scheduling to secure your appointment slot.
If payment is not received within 2 hours and alternative arrangements have not been approved by our office, your appointment will be canceled.
ESTABLISHED PATIENTS:
1) Schedule your appointment through the patient portal or website.
2) Return to our website and select the payment link that matches the appointment you scheduled on the service type page. Complete payment within 24 hours of your scheduled appointment.
If payment is not received within 24 hours and alternative arrangements have not been approved by our office, your appointment will be canceled.
If you are paying with a FSA card OR CareCredit, please state that within the appointment's note section at your time of booking. We will give you a call to collect your payment information.

Cancellation policy
General Healthcare Patients
Cancelled appointments must be made 24 hours prior to appointment time. If you are needing to reschedule, please do so within the patient portal or call the office for assistance.
We have a right to charge late cancellation/no-show fees each time an appointment is missed. This will be left to the provider's discretion.
SELF-PAY WEIGHT LOSS-REFUND POLICY
- Refunds for consults are non-refundable.
- Rescheduling of the appointment is acceptable at no extra charge if done so 24 hours prior to the appointment time.
- If you are booking an appointment less than 24 hours than the start time and want to cancel the appointment, a refund unfortunately will not be provided.
Lipodissolve, Hydration & Wellness Injections-REFUND POLICY
- Refunds for consults are non-refundable.
- Refunds for hydration infusions & injections are non-refundable.
- Rescheduling of the appointment is acceptable if done so 24 hours prior to the appointment time.
- If you are booking an appointment less than 24 hours than the start time and need to reschedule the appointment, please contact the office directly.
Cancellation policy will be honored for all services as listed above
Medications are 100% non-refundable
