How to Schedule Appointment
1)Specify whether you are a New or Established patient.
2)Select the appropriate appointment type from the drop down list.
Please select Specialized Exam from the appointment list.
An invoice will be sent to your email once you have scheduled your appointment. The Invoice MUST be paid 24 HRS PRIOR TO YOUR APPOINTMENT DATE. We will unfortunately have to cancel your appointment if the fee is not collected prior to your appointment.
IF SCHEDULING LESS THAN 24 HOURS IN ADVANCE, CONSULT/VISIT FEE WILL BE DUE AT THE TIME OF SCHEDULING APPOINTMENT.
We will call you to gather your demographics prior to your appointment date.

Telehealth Patients
Please make sure to complete the consent forms and client questionnaires prior to your scheduled appointment time.
You will be emailed an URL link from DocuSign to complete the required forms.
In-Person Patients
You will have the option to complete the required forms prior to your appointment date via the emailed URL link from DocuSign, or you may arrive 15 minutes early to your appointment and complete the forms in person.
General Healthcare Patients
Cancelled appointments must be made 24 hours prior to appointment time. If you are needing to reschedule, please do so within the patient portal or call the office for assistance.
We have a right to charge late cancellation/no-show fees each time an appointment is missed. This will be left to the provider's discretion.
