You have the option to complete the required forms prior to your appointment by clicking the consent button below, or you may arrive 15 minutes early to your appointment and complete the forms in person.
weight loss & Lipodissolve Patients
How to Schedule Appointment
1)Specify whether you are a New or Established patient.
2)Select the appropriate appointment type from the drop down list.
Please select New pt- WT loss OR Lipodiss from the appointment list.
- In the notes section, please state whether the consult is for Lipodissolve OR Weight loss.
An invoice will be sent to your email once you have scheduled your appointment. The Invoice MUST be paid within 24 HRS upon receiving the invoice. We will unfortunately have to cancel your appointment if the fee is not collected within the specified timeframe.
If you are paying with a FSA card OR CareCredit, please state that within the appointment's note section at your time of booking. We will give you a call to collect your payment information.
IF SCHEDULING LESS THAN 24 HOURS IN ADVANCE, THE CONSULT/VISIT FEE WILL BE DUE AT THE TIME OF SCHEDULING APPOINTMENT.

We offer self-pay options for individuals without insurance
Please specify in the notes section if you will be paying out-of-pocket. We will send you a $55 invoice which must be paid within 24 hours of receiving it. If your appointment is scheduled less than 24 hours away, the invoice fee is due at the time of receiving it.

Cancellation policy
General Healthcare Patients
Cancelled appointments must be made 24 hours prior to appointment time. If you are needing to reschedule, please do so within the patient portal or call the office for assistance.
We have a right to charge late cancellation/no-show fees each time an appointment is missed. This will be left to the provider's discretion.
SELF-PAY WEIGHT LOSS-REFUND POLICY
- Refunds for consults are non-refundable.
- Rescheduling of the appointment is acceptable at no extra charge if done so 24 hours prior to the appointment time.
- If you are booking an appointment less than 24 hours than the start time and want to cancel the appointment, a refund unfortunately will not be provided.
Lipodissolve Injections-REFUND POLICY
- Refunds for consults are non-refundable.
- Refunds for injections are non-refundable.
- Rescheduling of the consult appointment is acceptable if done so 24 hours prior to the appointment time.
- If you are booking an appointment less than 24 hours than the start time and need to reschedule the appointment, please contact the office directly.
Cancellation policy will be honored for all services as listed above
Medications are 100% non-refundable
